What Is PSA and Why Does It Go Up?
PSA (prostate-specific antigen) is a protein produced by prostate cells — both normal and abnormal ones. A blood test measures how much PSA is circulating in your body. The number rises when the prostate is enlarged, inflamed, irritated, or — in some cases — when cancer is present.
The critical point most patients aren't told: PSA is prostate-specific, not cancer-specific. Roughly 75% of men with a PSA between 4 and 10 ng/mL who undergo biopsy do not have prostate cancer. That's why understanding the context of your number matters far more than the number itself.

What PSA Level Is Considered High?
There's no single cutoff that applies to every man. Age, prostate size, and individual baseline all matter. That said, here are the general reference ranges urologists use:

These are guidelines, not hard rules. A PSA of 3.8 in a 45-year-old is more concerning than a PSA of 5.2 in a 72-year-old with a large prostate.
Common Causes of Elevated PSA (Most Are Not Cancer)
Benign Prostatic Hyperplasia (BPH)
An enlarged prostate — extremely common after age 50 — produces more PSA simply because there are more prostate cells. BPH alone can push PSA well above 4 ng/mL without any cancer present.
Prostatitis
Inflammation or infection of the prostate can cause PSA to spike dramatically, sometimes into the 20s or 30s. Acute bacterial prostatitis is the most dramatic cause; chronic prostatitis causes more modest, persistent elevation.
Recent Ejaculation
PSA can temporarily rise 24–48 hours after ejaculation. Labs typically recommend abstaining for 48 hours before a PSA draw — if you didn't, a repeat test may be warranted.
Urinary Tract Infection
A UTI or recent urinary catheterization can transiently elevate PSA. If you had a recent infection, your urologist may recommend repeating the test after treatment.
Prostate Biopsy or Procedure
Any instrumentation of the prostate — including a prior biopsy — causes PSA to rise. A baseline PSA should be drawn before any procedure, not after.
Prostate Cancer
Cancer is a real possibility that must be evaluated — but it's one cause among many. The goal of a urologic consultation is to determine how likely cancer is and whether further workup is needed.
Beyond the Number: PSA Refinement Tests
When your PSA is in the gray zone (roughly 4–10 ng/mL), additional tests can sharpen the picture before recommending a biopsy.
PSA Density
Divides your PSA by your prostate volume (measured on ultrasound or MRI). A PSA density above 0.15 suggests the PSA is disproportionately high for prostate size — a flag for further workup.
PSA Velocity
How fast your PSA is rising over time. A rise of more than 0.75 ng/mL per year is considered significant, even if the absolute number is still in the normal range.
Free PSA Ratio
PSA circulates in two forms: bound to proteins and free. A low free-to-total PSA ratio (below 10–15%) is associated with higher cancer risk. A high ratio is reassuring.
4Kscore / PHI Test
Blood tests that combine multiple PSA forms with clinical factors to calculate a personalized probability of high-grade prostate cancer. Can help avoid unnecessary biopsies.
Prostate MRI (mpMRI)
A multiparametric MRI of the prostate is now the standard next step before biopsy for most men with elevated PSA. It identifies suspicious areas, guides targeted biopsy, and can reassure when nothing suspicious is seen.
What Happens After an Elevated PSA?
Repeat the PSA
If there's any chance of a transient cause (infection, recent ejaculation, recent procedure), a repeat test 4–6 weeks later is often the right first move.
Urologic Consultation
A urologist reviews your full picture — PSA trend, symptoms, family history, prostate exam findings — and determines whether further workup is needed and how urgently.
Prostate MRI
For most men with a persistently elevated PSA, an mpMRI is ordered before biopsy. It identifies suspicious lesions and assigns a PI-RADS score (1–5) that guides the next decision.
Biopsy (If Indicated)
If MRI shows a suspicious lesion (PI-RADS 4 or 5) or PSA remains concerning without a clear benign explanation, a targeted biopsy is recommended. Not every elevated PSA leads here.
Active Surveillance or Treatment
If cancer is found, the grade (Gleason score / Grade Group) determines urgency. Low-grade prostate cancer is often monitored rather than immediately treated. High-grade cancer requires prompt action.
Questions Patients Ask About Elevated PSA
My PSA went from 2.1 to 4.8 in one year. Should I be worried?
A rise of 2.7 ng/mL in a single year is significant and warrants urologic evaluation. PSA velocity — how fast it rises — is often more informative than the absolute number. A urologist will look at your full trend, your prostate size, and any symptoms to determine the right next step.
Can I wait and repeat the PSA before doing anything else?
Often yes — especially if there's a plausible transient cause like a recent infection, recent ejaculation, or recent procedure. A repeat test in 4–6 weeks is reasonable in those situations. However, if your PSA is significantly elevated (above 10), rising rapidly, or you have urinary symptoms, waiting too long isn't advisable.
My doctor said my PSA is 'borderline.' What does that mean?
Borderline typically means your PSA is in the gray zone — elevated enough to warrant attention but not high enough to make the diagnosis obvious. This is exactly where PSA refinement tests (free PSA, PSA density, 4Kscore) and prostate MRI are most useful. A urologist can help you decide which additional tests make sense.
Do I need a biopsy right away?
Not necessarily. The standard of care has shifted significantly in the last decade. Most guidelines now recommend a prostate MRI before biopsy for men with elevated PSA. If the MRI is reassuring (PI-RADS 1 or 2), many urologists will recommend monitoring rather than immediate biopsy.
I'm on finasteride (Proscar/Propecia). Does that affect my PSA?
Yes — significantly. Finasteride and dutasteride (5-alpha reductase inhibitors) reduce PSA by approximately 50% after 6–12 months of use. If you're on one of these medications, your urologist needs to double your PSA value to get a true estimate. Failing to account for this can mask a rising PSA.
My father had prostate cancer. Does that change my risk?
Yes. A first-degree relative (father or brother) with prostate cancer roughly doubles your lifetime risk. Men with a family history should start PSA screening earlier — typically at age 40–45 — and may have a lower threshold for further workup at any given PSA level.
What if I don't want a biopsy?
That's a completely valid position to discuss with your urologist. Prostate MRI, PSA refinement tests, and active surveillance protocols exist precisely to help men avoid unnecessary biopsies. A urologist can help you understand your actual risk level and whether watchful waiting is a reasonable option in your specific case.
